Dr. John Leung has been a civil surgeon authorized by the U.S. Citizenship and Immigration Services (USCIS) to perform immigration medical examinations since 2016. Dr. Leung and his tailored team at Boston Specialists understand the stress accompanied by the immigration process and the importance of assisting individuals and their families to meet their goals quickly. He and his staff have processed over 4,000 applications and are extremely knowledgeable about the vaccine requirements set by the government.

Basic Info

Unconditional guarantees
- If our certified form is rejected for ANY reason, we will amend it free of charge within 3-4 business days.
- We will also price-match any USCIS-certified civil surgeons within 20 miles for the same service.
- We will accept a print-out of fees listed online. Price matches may be requested within 14 days of payment.

Step-by-Step Process
1. Book your appointment online. Choose “i693/Immigration” as the reason for your visit. Ensure that you have chosen the correct location for your visit.
2. You will then receive a confirmation email. Please follow the directions to upload all of the required documentation before your appointment. This will help the appointment run smoother!
3. Labs will be drawn at our clinic during your appointment per USCIS guidelines. Please bring your vaccine records for review by the Civil Surgeon team during the visit. Any missing vaccines can be obtained at our clinic/outside facility, or verified through antibody titer at our clinic.
4. Wait for an email notification from our team when your form is completed. Our typical processing time is approximately 14 days.
5. Return to the office to pick up your completed i693 form.

Locations
Boston Specialists primarily functions out of our main office in Boston. However, we are also able to offer medical examinations out of our Brighton location every other Wednesday for patients who may find the location more convenient! The addresses for both locations are as follows:

Prices

How much do you charge for the i693 application?
$380 for the first person, $360 for the second person, $340 for the third person, $320 for the fourth person and everyone else. Payments by card are subject to a 5% processing fee.

What does this fee include?
It includes a physical medical examination and the required labs. Any additional labs and vaccines are extra.

How much are vaccines?
The out-of-pocket cost for vaccines are $130 for Tdap, $105 for Polio, $230 for Varicella, $140 for MMR, $150 for Hepatitis B, $75 for influenza (during flu season between 10/01 - 03/31).
These prices do not include the 5% processing fee for card payments. We are unable to accept insurance for immigration vaccines.

How much are the antibody titers?
The out-of-pocket cost for titers are $90 for MMR (measles, mumps, rubella) titer, $70 for Hepatitis B titer, $50 for Varicella titer. We are unable to accept insurance for immigration labwork.

How do I know that you are not overcharging me?
We price-match any online listed price from a USCIS-certified civil surgeon within 20 miles of our clinic.

How long does it take to receive my sealed i693 form?
Our typical processing time is approximately 14 days.

Can you expedite my form?
We are always working tirelessly to return every patient's form as fast as we possibly can! If you have a USCIS deadline, please let us know at the time of your appointment and we will accommodate the best we can.

Can I receive an unsealed copy for myself?
Yes! You will receive an electronic scan of your entire i693 form via email when your form is ready for pick-up. We can provide an extra printed copy for $10.

Can I use insurance to pay for the labs and vaccines?
Unfortunately, the answer is no. Insurance companies will not cover the lab tests or vaccines obtained for the purposes of the immigration application. The standard fee we charge already includes all the required labwork (Quantiferon, RPR, Gonorrhea).

Do you have an interpreter?
No! If you are non-English speaking, please bring your own interpreter to your appointment. They will sign as your interpreter on your i693 form. Our clinic staff are unable to act as medical interpreters.

What if I don't have any vaccine records?
No worries. Our Civil Surgeon team will help you through the entire process! For more information, please see the Vaccines section.

Will you notify me when my form is ready? How will you notify me? Can you let me know as soon as possible?
Yes! We will notify you by email as soon as your form is ready!

How do I pick up my form? Will you ship to me?
For Chinatown patients, we offer pick-ups at our 65 Harrison Ave location during select business hours (will be further specified in email). We can also ship your form to you for $35 + USPS shipping fee. Please see Shipping section for more info.
For Brighton patients, we offer pick-ups at 77 Warren Street on select Wednesdays morning and at 65 Harrison Ave location during select business hours. We offer shipping priority shipping for Brighton patients. Please see Shipping section for more information.

Labwork

What labs do I need?
Generally, you will need QuantiFERON, RPR, and Gonorrhea labs. If you are younger than 18, you only need the QuantiFERON.

Where can I get the lab done?
All the lab tests will be done in our office with QUEST Diagnostics, which is included in our standard fee package.

Can I use insurance to pay for the labs?
Unfortunately, the answer is no. Insurance companies will not cover the lab tests obtained for the purposes of the immigration application. The standard fee we charge already includes all the required labwork.

I did the same lab work last year. Can I use those results?
The immigration medical exam is supposed to be a “snapshot” of your health. The labs need to be ordered by the civil surgeon directly.

I have HIV. I heard it may delay the completion of the application. Is it true?
Patients with HIV may experience a delay as government guidelines require a chest x-ray and referral to the Public Health Department for three sputum smears and culture to rule out active tuberculosis. However, we have experience working with HIV-positive patients and will guide any patients with HIV to obtain the correct information for their application!



Vaccines

Do I need to get the COVID-19 vaccine?
Beginning July 2022, the United States requires that all applicants 6 months and older must receive the required vaccinations against COVID-19, unless medically exempt. Please bring proof of your Covid-19 vaccination to your immigration medical exam. Click here to be redirected to the USCIS website for more information.

What can I expect if I don’t get the COVID-19 vaccine?
If an applicant refuses a COVID-19 vaccine series in part or entirety:
“If an applicant refuses one or more doses of a COVID-19 vaccine series that is medically appropriate for the applicant, it should be documented that the vaccine requirements are not complete and that the applicant refuses vaccination. This applicant is Class A and is inadmissible to the United States.”

What vaccine do I need?
It depends on your age and medical conditions. We will obtain a detailed medical history during the visit and inform you what vaccines you need based on CDC guidelines.

Where can I obtain the required vaccine(s)?
You can obtain the vaccine(s) at your PCP office and local pharmacy. For easier access, you can also obtain them at our clinic on the same day with additional costs.

Will my insurance pay for my vaccine?
Most likely. We will tell you what vaccine(s) you need and in what sequence. Then, you can call your insurance to find out which vaccine(s) will be covered.

If insurance does not cover the vaccine(s), how much will they cost?
It depends on what vaccines you need. Once Dr. Leung determines what vaccine you need, we will provide you with the price. You can also call your local pharmacy.

If my insurance pays for my vaccines, can I get them at Dr. Leung’s office?
No. We are unable to bill your insurance for vaccine(s) as your civil surgeon. We encourage you to go to your PCP’s office or local pharmacy to get the needed vaccines if your insurance covers them. Dr. Leung will give you the prescriptions. You can also get the vaccines at Dr. Leung’s office if you are willing to pay out of pocket.

I lost my childhood immunization records? Is that a problem?
For immigration medical exam purposes, if it is not documented, it never happened! But it is a-OK. For some vaccines, For some vaccines, we can check the antibody level in your blood as proof of immunization. For others, we may have to re-administer them.
