Civil Surgeon
Dr. Leung is a civil surgeon authorized by U.S. Citizenship and Immigration Services (USCIS) to perform immigration medical exam since 2016. He and his team at BFAC understand the this process can be stressful and are experienced with helping individuals and families meet their goals. Dr. Leung and his staff have processed hundreds of applications and are very knowledgable about what vaccines you need to fulfill government requirements. Our goal is to complete your vaccination record based on up-to-date government guidelines. BFAC strictly adheres to the civil surgeon guidelines and the CDC guidelines published by the government.

How to make an appointment?
(617)804-6767 or online booking (chose “Immigration Medical Exam” as the reason for visit).
What is the process?
The process is simple. You call us and we will tell you what vaccines and lab you will need, based on your age and medical conditions. We will tell you where to obtain the labs and vaccines, and schedule a date and time for you to come to our office for the required physical examination. You will be able to to pick up the certified i693 form on the same day when Dr. Leung conducts the physical examination, so you only have to come to office once.

Yes. At Boston Food Allergy Center the safety of our staff and patients is our top priority. We have implemented many safety measures to ensure your safety during your visit.
Do you offer virtual visit for the immigration?
Yes. We first do a virtual interview to find out what lab and vaccines you need. You only have to come to office once to do a physical examination and pick up the certified forms in sealed envelope

How much do you charge for the application?

What does this fee include?
Medical examination only. Labs and vaccines are extra.
How much do you charge for labs if I want to pay out-of-pocket?
The out-of-pocket lab fee is $160 for patients 15 yo and older, $90 for patients younger than 15.
How do I know that you are not overcharging me?
We price-match any online listed price from a USCIS certified civil surgeon within 20 miles of our clinic.
Labs
What labs do I need?
If you are 15 or older, you need quantiFERON, RPR, and gonorrhea labs. If you are younger than 15, you only need the quantiFERON.
Where can I get the lab done?
During the virtual interview, we will send you an order for the labs. You can obtain your labs at your PCP’s office, commercial labs, or at our clinic.
Can I use insurance to pay for the labs?
Most insurance companies will not cover labs obtained solely for the purpose of an immigration application. You can always call your insurance to find out. Our office will not be able to tell you about your specific insurance coverage.
I did the same lab work last year. Can I use those results?
The immigration medical exam is supposed to be a “snap shot” of your health. The labs need to be ordered by the civil surgeon.
I do not have insurance. What is the most affordable way to pay for the labs?
Thanks to the high volume of applicants we serve, we are able to negotiate with Quest Diagnostics for a significant discount. Our lab fee is $160 for 15 or older, and $90 for younger than 15. To further ensure our prices are the most affordable, we price-match any price listed online by a certified civil surgeon within 20 miles of our clinic.
I have HIV. I heard it may delay the completion of the application. Is it true?
Patients with HIV may experience a delay as government guidelines require a chest x-ray and referral to the Public Health Department for a sputum culture to rule out active tuberculosis.
Please Note: If you choose to obtain your labs elsewhere, please make sure the outside lab does not alter the orders Dr. Leung prescribe to you. For example, it is required that a lab known as “RPR” to be checked. RPR is a test to screen for syphilis. More often than not, when the applicant takes the RPR lab order to an outsider lab, the outside lab may change the order to a newer generation of syphilis test that looks for a specific syphilis antibody. The US government will not accept those newer generation antibody tests as the first screening test.
Vaccines
What vaccine do I need?
It depends on your age and medical conditions. We will obtain a detailed medical history during our virtual interview and tell you what vaccines you need based on CDC guidelines.
Where can I obtain the required vaccine(s)?
You can obtain the vaccine(s) at your PCP office and local pharmacy. You can also obtain them at our clinic if you are paying out-of-pocket.
Will my insurance pay for my vaccine?
Most likely. We will tell you what vaccine(s) you need and in what sequence. Then, you can call your insurance to find out which vaccine(s) will be covered.
If insurance does not cover the vaccine(s), how much will they cost?
It depends on what vaccines you need. Once Dr. Leung determines what vaccine you need, we will provide you with the price. You can also call your local pharmacy.
If my insurance pays for my vaccines, can I get them at Dr. Leung’s office?
No. We are unable to bill your insurance for vaccine(s) as your civil surgeon. We encourage you to go to your PCP’s office or local pharmacy to get the needed vaccines if your insurance covers them. Dr. Leung will give you the prescriptions. You can also get the vaccines at Dr. Leung’s office if you are willing to pay out of pocket.
I lost my childhood immunization records? Is that a problem?
For immigration medical exam purposes, if it is not documented, it never happened. But it is a-OK. For some vaccines, we can check the antibody level in your blood as a proof of immunization. For others, we may have to re-administer them.
